Sir,
With due respect , I XYZ policy holder (Policy number:-xxxxxxxxx) in your insurance company had gone through medication and diagnosis due to frozen shoulder. I was prescribed by Dr.Xxxxxx to have a surgery .The expenditure for my medication and diagnosis was approximately Rs-Xxxxxx.
I have gone through my health policy and it states that i am eligible for this claim. So i request you to please look in this claim settlement process . I am attaching photocopy of bills which got during medication and diagnosis and I am also attaching my policy details. I would appreciate your efforts if you ponder my request.
