Respected sir,
This is to inform you that the name of my wife, (Name) was incorporated in my Policy No. Xxxxxxxx as nominee. I regret to inform you that she expired on xx/xx/xxxx as per the death certificate dated xx/xx/xxxxenclosed. Since I have again married after the -death of my wife, I have felt the necessity of getting the name of my newly married wife incorporated in my above Policy as nominee.
In this connation, I am enclosing consent letter dated xx/xx/xxxx of my wife (name of my wife) desiring to become nominee in my Policy.
I would now request you to arrange to incorporate her name in my above Policy as nominee after deleting the name of my deceased wife from the Policy and inform me having done so.
